Program tool 22. Program log forms

Log form for public awareness in your legion or centre

This log form allows you to keep track of the information you share with your members or post at your legion or centre. Each time you put up a poster related to falls prevention or active living keep a copy for your records.

Write down the name of the pamphlet that was shared or the poster and what was put up.
